Hi, I am brand new here. I am curious. I hope this is okay to ask. Do you work for a hospital or clinic in house? Do you work for a hospital or clinic from your home? Do you contract yourself to a hospital or clinic on your own? Do you work for an out source company? If you work for an out source company, do you work from an office or from your home?
No reason for any of this, just wondering and trying to get to know the board and you!
I work from home. I have a question. When you are working as an independent contractor do you list on the W9 form their business name or your business name that you would like your checks made out to?

The person doing the paying is the one that fills out and submits the W-9 form. You can instruct them to use your name or your business name, but it must match that of the checks paid to you.
