This has been asked, and opinions given, many times.
Only you can decide what you are going to charge a new account, whether it includes templates or not, and the size of the template, to me, is irrelevant.
When using templates, you have to figure the time spent in making the template, maintaining the template, modifying the template, calling up the template, and so forth. You also have to factor in whether or not you think the provider will pay for the whole template or not.
If she has not said anything about not paying for the template, make your offer counting it and negotiate downwards if necessary. This is a lot easier than offering the contract price without the template and learning later that the provider frequently strays from the template or makes changes to the general template over time.
JMO, and you will probably get many different opinions.
