If dictated 40 to 60 mg, does it need to be transcribed
40 mg to 60 mg, or can you just say 40-60 mg or 40 to 60 mg?

It depends on what style guide you follow. BOS 2nd edition would say 40-60 mg or 40 to 60 mg would be acceptable. I don't have the BOS 3rd edition, so have no idea what that says. The SI/NIST convention would be that you must include the mg with each dosage and not use a hyphen. I know the BOS 3rd edition decided to recommend conforming to SI/NIST conventions where use of a hyphen between number and unit in a compound adjective was concerned, but I have no idea whether they decided to conform to the rest of the rules. Perhaps someone who has purchased the 3rd edition can comment on that.

I can throw my 2 cents in... being a nurse, i would ask "is the MD increasing 40 what to 60mg". Is it 40 mg(probably) or units or kg or ml. It is just a case of CYA. I always make sure I have a measurement behind any dosage.

Generally, I don't recommend that a measurement be added if not dictated. To me, that's a better way to CYA - not adding medical information that is not specifically dictated. As a general rule, 40 to 60 mg would be perfectly appropriate in transcription.

I would agree; unless there is absolutely no question what the unit of measure must be, don't add it. If you think it is essential there be a measurement behind every dosage, then the last thing you should do is assume you know what that measurement is. If it's missing, type just the number and flag it to the dictator's attention.
