When should a heading in a report be on a seperate line or used as a paragraph heading.

CLINICAL INFORMATION: Left hip pain. blah blah blah blah

IMPRESSION
Mild interval improvement, blah blah blah

I see reports using both formats. I know when items are bulleted or numbered the heading should be alone on the first line but after that there doesn't appear to be a set rule.

That's because there isn't a set rule. Clients use a wide range of different formats for reports. Some like the space savings or the look of placing headers on the same line with the information in that section, some prefer having the headers on a separate line, and some even mix the styles, using both in different areas of the document or placing headers for major sections on separate lines and subheadings on the same line with text. Follow client preference, or if there is no client preference, just use your own preferred style guide or your own best judgment.

Asking what the one set style is for medical documentation is like asking what the one set style is for a resume or a business letter. There are several recognized formats and variations on the themes. Some are more popular than others, although which is the most popular tends to vary over time, and a less popular layout may be more suitable to a particular resume or letter. In addition, there may be only one acceptable format for a given company which you are expected to use if applying to or working for that company. That doesn't make it the one and only style or even the best style, just the one you need to use for that company.

Absolutely. I transcribe for clinics in three different states, and I usually request templates so I'll know what they want. Some dictators vary within the same clinic. Some like their meds listed and numbered, some want it in paragraph format. So it's always best to find out what they want.
