YES, this is tongue-in-cheek humour.
But the secret reason why all white people love socialized medicine is that they all love the idea of receiving health care without having a full-time job. This would allow them to work as a freelance designer/consultant/copywriter/photographer/blogger, open their own bookstore, stay at home with their kids, or be a part of an Internet start-up without having to worry about a benefits package. Though many of them would never follow this path, they appreciate having the option.
If you need to impress a white person, merely mention how you got hurt on a recent trip Canada/England/Sweden and though you were a foreigner you received excellent and free health care. They will be very impressed and likely tell you about how powerful drug and health care lobbies are destroying everything.

Tongue-in-cheek humor is "deadpan" humor. "Pan" is a show-biz slang term for face or dramatic mask, so deadpan means an expressionless face. Tongue-in-cheek humor is straight-faced humor, implying that you say something with every outward sign that you are serious-- but you're saying the opposite of what you mean, and that's the source of comedic effect.
High deadpan humor employs such a good imitation of serious demeanor that it is often misunderstood or missed altogether. Often those being talked to are not "in on the joke," and certain onlookers, privileged friends of the "comedian," are being entertained at the expense of the dupes that take the tongue-in-cheek remarks seriously.
I don't mean to downgrade this type of humor-- it's the hardest to do well, and some of the finest humor is blandly facetious in nature. But it's a very easy humor to do badly-- all you have to do to practice "low" deadpan humor is tease people by saying things you don't mean. If your targets are fooled and your friends are in on the joke, you're at least funny to your friends-- if they're undiscriminating people, and the friends of low-humor "wits" often are, all you have to do, to share a laugh with them (later of course) is to baffle and annoy people.
