Hi,
How is this done when they give a range such as 120 to 140 over 80 t0 95?

120-140/80-95? 120/80 to 140/95?

When it is dictated like that, I transcribe whole thing in words, "120 to 140 over 80 to 95." (transcribe what the doc says and let someone else make the guesses as to what he means!)

Your second example makes a invalid assumption--the low systolic doesn't necessary go with the low diastolic, for example.
