It galls me to no end when you hear water running in the background and then a flushing sound.
Do they think no one is listening or that we are too stupid to figure out what's going on?

I'd have to say the ones who *cough* attempt to spell things.
A close second would be the hemmers and hawers. I recently posted about an ESL doc who does this annoying "um-num-num" sound when he pauses...so annoying.

How about the ones who are heavy breathers throughout their MANY pauses? Or, they cough (which always hurts my ears) while they are saying a word, so you can't understand it, but they never repeat the word? OR, the docs who spell every patient name, Smith, Jones, until they get to Giovankalski, and then assume that you know how to spell that one!?

I agree totally with inability to use the pause button. 
I had to listen to a dictator this morning who was doing that and at the same time he was tapping the keyboard and saying, "This f*ing thing takes so long to load." (I guess he thought I couldn't hear it or I could do something about it.) Seems to be okay to make ME wait, but heaven forbid his waiting for anything.

Speaking of coughing, I have one that talks so fast that when he stumbles over a word, he tries to cover it up by clearing his throat, which wouldn't be so bad, except for the fact that you can hear him producing a wad of phlegm along with it. I swear he's going to let one fly across the room one of these days. Yuk.

How about nose-blowers? I dont' want to see it or hear it. I believe that everyone should blow their noses in private. Seeing it or hearing it is disgusting!

I don't believe I have had any nose blowers...yet.
Another one I hate is when the doc is sitting right next to his phone, and they all have the most obnoxious, ear-splitting rings, and just keeps talking throught it instead of silencing it, never mind repeating what he just said!

This one I loved: The overhead goes off i.e., "paging Dr. Whats-His-Name..." and the dictator says, "Typist, please don't type that."
First of all, calling me a typist was a very bad move on his part. Then, to follow it up with the assumption that I would actually include into the report, an overhead page which had absolutely nothing to do with the patient was more than I could handle. Dr. Idiot, I presume?

I don't get it either when they do that with the tough patient names, since they should typically have it written down in front of them, but I'm usually willing to give them a little bit of grace if they're talking about the referring doctor. I don't think they known how to spell some of their colleagues' names any more than we do without looking them up.

I had one the other day who must have been holding his baby while dictating...and the little darlin' was screaming at the top of his lusty little lungs through the whole dictation. The doc never missed a beat. I sincerely hope he enjoyed filling in all the Swiss cheese holes in that report!!

I have one lady who does her dictations on a speaker phone as she strolls around her office doing who only knows what. Her voice gets loUDER..HER VOICE GETS SOfter. loUDER...SOfter. She rifles through papers right next to the phone, then she runs the shredder from across the room. I hear doors opening and closing. There is also the possibility that she has a microwave in the room. Ah, the beauty of multitasking!

I have several dictators who do that, too, and it's especially tough with one in particular because he's an ESL. I have another ESL who yawns and keeps right on dictating. I also hate it when the orthopods hold the x-rays right up by the recorder and shake them around. No, I CAN'T hear you now!

I can handle fast talkers, ESLs, mumblers, munching on potato chips, etc., but the one thing that absolutely drives me over the edge is humming and hawing over every single sentence..."Ummmmmmm...(2 minutes go by)...the patient's blood pressure is...Um...(heavy sigh)...(the sound of pages flipping)...(2 more minutes go by)...120/75...ummmmmmm...
Just shoot me now.
