How about this one:
His PAST SURGICAL HISTORY includes deviated septum repair, left shoulder replacement, right eye removed and he is blind in the right eye...
Gee, ya think?

"The patient has a history of tinnitus. He describes it as an 'oosh-oosh' sound. That's spelled o-o-s-h space o-o-s-h."
Gosh, I sure wish this doctor would spend more time spelling the REAL words she so badly mangles. lol

Good thinking there, doc!

Doc actually dictated this - "...the whole sequence of events was outlined in my 03/24/08 note so, thankfully, I don't have to regurgitate all of that again."
To add insult to injury, he had hiccups at the time he was dictating, so it sounded like he actually would regurgitate!

Dictated: 20 mg 3 times per day. Transcribed by speech recognition: 20 meatballs 3 times per day.
Well, we KNOW the patient won't be hungry!!

Patient was operated on, well on some date. I don't remember operating on her.
Patient is awake, alert, oriented, asleep on the floor.

The patient is a male with a very short attention span, who is very difficult to get a coherent history from. (Dictation ended here....um, the patient has a short attention span?)

The patient is a S/L "cute", 39-year-old male....
I left a blank. I wanted no part in that blank. Let QA earn their $.

I have a new ER doc today....he keeps calling me "dictationist," as in, "Dictationist, could you please go up and change my abdominal exam to...Thanks, dictationist."
