I know how to punctuate these, but for some reason I am questioning myself tonight. I looked to see if I could find a similar post and can't find anything that fits this question. When transcribing something like..."A healthy-appearing 28-year-old Hispanic female" For some reason tonight I am thinking that there should be commas in this, but when looking over previous reports, I have never put one in????

Go down to the table on the Royal Order of Adjectives and the comments below it on using commas only when adjectives in a series belong to the same descriptive category (coordinate adjectives).
In your example, healthy-appearing is an adjective of observation, 28-year-old one of age, and Hispanic one of origin. Since none of them belong to the same descriptive category, they are cumulative adjectives, and you put no commas between them.

That's what I thought. I am just questioning myself for some reason. I guess I don't transcribe too many with that many adjective in a row. I will have to bookmark that website. Thanks!

Someone once told me that unless you could put "and" in between there, you don't need a comma.
You wouldn't be able to put "and" in between any of those, therefore, no commas.
(I have a pea brain, simple works easier for me! LOL)

Yeah I heard that one too....that makes it much easier. When reading the website 14tonks referred me to, it does say that if you cannot place "and or but" in between the adjectives, they don't belong there. It does make sense. I hate it when I start second guessing myself on something I do all the time. Sometimes with the monotony you just do things out of habit, but when you actually stop and really think about it instead of just typing, you start to question yourself. Thanks for the help!

How would you go about puntuating these two sentences. I think I know where the commas would go but for some reason I am questioning myself. "DUH"!!!
Scrapings are taken from two ulcers with a #15 blade and smeared and stained immediately by the Tzanck technique with toluidine O.
SUBJECTIVE: This is a 19-year-old single white female college student who is seen with reference to vulvar pain and urinary burning of 2 to 2 1/2 days duration

Longtime lurker here and wondering if someone can confirm my logic with The Royal Order of Adjectives (a great help, thank you, 14tonks).
1) widespread erythematous papules - no commas (observation and physical description)
2) soft, freely mobile nodules - comma (observation and observation)
3) firm pink scaly papule - no comma? (observation, physical description, and observation?)
4) pink swollen turbinates - no comma (physical description and observation)
5) mild, intermittent asthma - comma (observation and observation)
6) a biopsy-proven, superficial, multicentric basal cell carcinoma - commas (qualifier?, obervation, and obervation)
Thanks for any advice.

Just wondering why there has been no response to my request above. Should I have not posted in this thread but started a new thread? Am I supposed to have figured out the answers to my questions based on information already given in this thread? Or maybe because I'm a new poster?

The Grammar forum isn't widely traveled, actually. However, generally speaking it's best to start a new post for a new question to make sure we catch it.
As for your actual question - perhaps Tonks will see it and might have time to critique it based on the ROA. The commas look okay to me, but there are definitely people better at the finer points of grammar than I am.

Sometimes, it's difficult to classify adjectives in one category or another in order to determine whether they're cumulative or coordinate. To me, all the adjectives in your third example are of the same type (descriptive?). I'd probably transcribe "firm, pink, scaly papule." The others look fine to me.
