I do feel your pain. I was not in the best of moods last week because of a dictator that was like the one described by Deej. This fellow would run through sections without breathing or even saying each word. He would inhale on medicine dosage and not even pronounce the dosage. Have you ever seen a fish net? That's about how many holes I left in his reports. I got stuck with five or six of his back to back. I was ready to sit down and cry. I finally just finished the last report and went to bed. Thank goodness for national companies. It may not have been the ethical thing to do but for me, it was the sane thing to do.
I did send a message to the office in regards to this particular doc and was given the spill that his reports were basically canned or stock phrases and that I would eventually understand them. Personally, I don't really feel this is good enough because we are dealing with patient records for heaven's sake. But, I do have to acknowledge that I'm a new mt and the ones in charge do have experience that I don't possess yet. I will say that if I was working for myself, I wouldn't do those reports if I got triple pay for it. It's just not worth the headache.
I'll step off my pity potty on this one before I fall in. 
I have to say though in defense of the company I'm working for that out of all the doctors I have transcribed work for, there are only about three that I absolutely do not like doing their work. To me, the reason this impresses me is because the accounts I am in are for not only a teaching hospital but is also one, as of last time I checked, that was listed something like second best in the nation just under Johns Hopkins.
When I take into consideration the prestige, teaching programs, and the advance medical treatments and surgeries, it really is amazing that there aren't more terrible dictators. Doctors come from all over the world to practice or learn there. This is one way I have of reminding myself that it could be worse.
