I transcribe for a doctor and he is not very clear on the titles for Mrs. Ms. Miss. He does not use them regularly, I don't think anyway. There have been times where I know that he just says Ms. (in fact, he mostly uses that)But then when I do another report on the same person he will say Mrs. How do I know without any charts. Should I just use Ms.

I might be confused, but I understand that you are not supposed to use the patient's names in the notes. Maybe that only applies to certain types of records.

In the past, before we stopped using the patient names, I always put Ms. if I could not prove the patient was married.

In hospital reports, the patient's name is not supposed to be used. But in clinic and office reports, the names are used frequently. If the doctor dictates discrepancies between Mrs. and Miss or Ms., I just use Ms., because, as you say, I have no way of knowing which is correct.
